Parent Guardian Consent

As the parent or guardian of an employee under 18 years of age, Midfield Group requires your written consent to the following:

  1. Employees of the Midfield Group may be required to undertake a Medical Examination to confirm suitability for employment.
  2. Midfield may make deductions from wages to where the employee incurs costs and chooses not to pay by cash.

Medical Examination Agreement

The following requirement are applicable to the employee under 18 years of age:

I agree:

  1. That a medical examination may be carried out

a) Prior to employment at The Midfield Group P/L

b) During the course of employment at The Midfield Group P/L

  1. That such a medical examination may include any of the following supervised tests – breath, blood, urine, hair, oral fluid or other method depending on the technology available.

  2. The empoyee shall make themselves available at a nominated testing facility for such medical examination no later than 24 hours from this form being signed by The Midfield Group HR Manager.

  3. That the results of this examination will be forwarded to the The Midfield Group HR Manager, Midfield and shall be held in strict confidence.

  4. The results of the authorised testing agency shall be accepted as being final.

  5. I give permission for Midfield Authorised Officers to discuss drug and alcohol counselling, education and assistance in rehabilitation with WRAD (Western Regional Alcohol & Drug Centre Inc.), Healthscope, St John of God Hospital and or other medical personnel involved in the testing or rehabilitation phase.

Deductions from employee wages

The following requirement are applicable to the employee under 18 years of age:

The Midfield Group proposes to make the following deductions from your wages where you chose not to pay by cash:

  • Deduction for the cost of food purchased by you from the Canteen at work. Deductions wil be passed onto the canteen operator – Canteen Management Services.

  • Deduction for the cost of products purchased by you from The Meat Barn. Deductions will be passed onto the Meat Barn operator – Meat Barns Australia.

  • Deduction for the cost of tools associated with your trade e.g. knives, steels, pouches, chains, stones and the like purchased by you from Midfield Meat International.

Midfield will make multiple deductions from your wages to cover the cost of any food, meat or tools purchased by you. If you do not purchase any food, meat or tools, Midfield will not make these deductions.

The deductions will commence from the date you sign the authorisation below and end on the date of your termination pay when you cease employment with Midfield. The deductions will be made from your weekly wages.

I authorise my employer Midfield to make multiple deductions for an amount as varied from time to time from my wages to cover the cost of any food purchased by me from the canteen, Meat Barn or Midfield Meat International where I have not paid by cash.

I authorise these deductions to be made on a weekly basis. I understand that if I have not purchased any food, meat or tools in the preceding week, no deduction will be made.

I authorise these deductions to be made during the period commencing on the date I sign this authorisation and ending on the date of my termination pay when I cease employment with Midfield.
