Parent Guardian Consent - Employment Contract

As the parent or guardian of an employee under 18 years of age (dependent), Midfield Group requires your written consent to the following:

I acknowledge that I have read and understood the Employment Contract provided to my dependent.
I hereby give my informed consent for my dependent to enter into employment under the terms and conditions outlined in the Employment Contract.
I understand that by providing my consent, I agree to my dependent’s participation in employment with The Midfield Group and acknowledge the responsibilities and obligations set out in the Employment Contract.
